Death Records are obtained from the Social Security Administration and provide the following information:Decedent's Name,Date of Birth, Date of Death, Social Security Number, City and State of Residence, Last Residence Zip Code, Zip Code of the Death Payment Recipient (Lump Sum Payment Zip), Verified or Proof Information (Verified indicates that the death information was verified with a family member or someone acting on behalf of the family; Proof indicates that a death certificate was observed.)
